Meet our Long-Term Care Providers

John w. richard, md


Dr. John Richard is the Medical Director of Bluegrass Extended Care Services. He is Board certified in Family Medicine and brings more than 14 years of experience in Geriatric and Family Medicine to Bluegrass Extended Care Services.  

Prior to completing his medical degree at the University of Kentucky College of Medicine, Dr. Richard also received undergraduate and graduate degrees in Civil Engineering at U.K. He fulfilled his Family Medicine and International Healthcare Residency at the University of Cincinnati. From 1998 to the present time he has continued his practice in Long-Term Care medicine.  Dr. Richard was previously employed by Lexington Clinic at Palomar Family Practice and now devotes his expertise with the team of providers of Bluegrass Extended Care Services.

Dr. Richard moved to the Bluegrass as a young boy and is proud to call it home. He and his wife have four sons. Dr. Richard’s free-time activities include his family, the outdoors, traveling, and hobbies involving cars and motorcycles.

Dr. Richard says: “I truly believe there’s something special about the people in the Bluegrass and I’m so thankful for my patients here. They’ve made me part of their families and their daily lives. It’s a very rewarding place to practice medicine.”

jennifer hardin, pa-c

Jennifer Hardin is a Physician Assistant with Bluegrass Extended Care Services. Her responsibilities include  assessment, diagnosis and treatment of chronically ill patients in the long-term care setting. She is certified by the National Commission of Physician Assistants, and licensed in the state of Kentucky by the Kentucky Board of Medical Licensure.  Since June 1998, she has been in practice as a Physician Assistant in the Lexington area.  

Jennifer received a bachelor’s degree in Physician Assistant Health  Sciences in June 1998 from  the University of Kentucky, and initially worked in a cardiology practice for three years.  In 2002, she began practicing in the long-term  care arena  where she now has been practicing for ten years.  
Prior to pursuing her degree in Physician Assistant studies, she received a bachelor of science degree  in Dietetics in 1989 and worked for seven years in the hospital setting as a clinical dietitian.  

Jennifer is fromMorehead, Ky. She moved to Lexington in 1998 to pursue career opportunities as a Physician Assistant.  She is married with one daughter and in her free time enjoys traveling, the beach, and spending time with her family.  

 

stacy mills, aPRN

Stacy Mills is an Advanced Practice Nurse Practitioner with Bluegrass Extended Care Services. She is certified with the American Academy of Nurse Practitioners and has more than six years of experience. Stacy received her undergraduate degree in nursing from the University of Kentucky and her Master’s degree from Eastern Kentucky University.  After serving two years in Barbourville, Ky. at a rural health clinic, she joined Dr. Richard’s team at the Lexington Clinic in 2008.  That collaboration continues today with Bluegrass Extended Care Services.

Stacy was born and raised in Lexington. She has one daughter and two sons.  In her off time, she enjoys gardening, cooking, and watching movies.

Janet Flinn, aPRN

Since May 2012, Janet Flinn has been a member of the Bluegrass Extended Care Services team as an Advanced Practice Registered Nurse. She graduated with a Bachelor of Science degree from the Eastern Kentucky University Nursing Program in 1981. Her practice as a registered nurse includes experience in acute care in medical/surgical nursing and cardio-thoracic ICU nursing.

In 1997, Janet received a Master of Science degree in nursing. Since that time, she has served as a family nurse practitioner in the family practice setting caring for patients across the lifespan with acute and chronic illness with a focus on preventive care. Janet has been Board Certified with the American Academy of Nurse Practitioners since 1997 and is a current member of Kentucky Coalition of Nurse Practitioners.  She has also been a preceptor for Nurse Practitioner students from Frontier University, Eastern Kentucky University, and the University of Kentucky over the past four years. In addition to her nursing degrees, Janet is also ACLS/BLS certified.

Originally from Louisville, Janet has lived in Lexington since 1993. She and her husband have two college-age children. She enjoys spending time with family and friends, working out, travel (especially beach destinations), U.K. athletics, and community involvement.

Joan osborne, APRN

Joan Osborne is an Advanced Practice Nurse Practitioner with Bluegrass Extended Care Services.  Her responsibilities include providing  comprehensive health assessment, diagnosis, treatment, management and evaluation of acute and chronic illnesses; provision of end-of-life care, pain management and family and patient education.

Joan has been Board Certified by the American Nurses Credentialing Center(ANCC) as a Family Nurse Practitioner(FNP)  since 1999, and is licensed by the Kentucky Board of Nursing (Ky BON) to practice as a Nurse Practitioner.

Joan received her RN, Midwifery Certification, and a Bachelor of Science degree in England, U.K., and her Master of Science degree from the University of New Jersey.  She has worked in England, the Caribbean, and  now, Kentucky.

Since arriving in the US, she has  worked as an Obstetric Nurse, as a Supervisor in sub-acute and rehabilitation facilities, as Director of Nursing for a county correctional and juvenile facility, and, since 2001 as a Nurse Practitioner in long-term care/geriatric care.

Joan enjoys traveling and sightseeing  around the US and abroad; reading; knitting; anything water related, especially snorkeling , canoeing and rafting;  walking/hiking; and theatre productions.

michelle jones, pa-c

Michelle Jones is a Physician Assistant with Bluegrass Extended Care Services. She is certified by the National Commission for Certification of Physician Assistants with special recognition in Primary Care and is licensed by the Kentucky Board of Medical Licensure.  She has been practicing since 1989.

She received a degree in Health Science from the University of Kentucky and has experience working in the fields of Internal Medicine, Neurology, Infectious Disease, and Hospital Medicine before changing to Long-Term Care Medicine in 2009.

Michelle was raised on a dairy farm in Hardin County, Kentucky , but currently lives in Jessamine county with her husband and three daughters. She enjoys power walks, crime novels, and family time.

“Long-term care and rehabilitation medicine can be challenging, but it is a very rewarding field. It’s my goal to improve my patients' quality of life in some way with every encounter while giving them the compassion and respect they deserve,” Michelle says.

Stacy Ramey is an Advanced Practice Nurse Practitioner with Bluegrass Extended Care Services.  Stacy’s responsibilities include providing quality patient care through assessment, diagnosis and treatment of chronically ill patients in the long-term care environment.  She is Board Certified through ANCC as an Acute Care Nurse Practitioner and she has been practicing in long-term care since 2005.

Stacy received her B.S. in Nursing degree  from the University of Kentucky in 2000 and her M.S. in Nursing in 2004, also from U.K.

Stacy enjoys traveling, music, shopping, vegetarian cooking, photography, and spending time with loved ones.

“Being an APRN is a very rewarding experience and I am glad to be part of this profession”, says Stacy.

 

Meet our Staff

Crystal Davis, Medical Assistant

 

Crystal Davis is Medical Assistant for Bluegrass Extended Care Services.  She provides general office management services, performs accounting and billing functions for the office and provides transient medical support to our providers.

Crystal has over 10 years experience in the healthcare industry, coming to Bluegrass Extended Care from the Lexington Clinic, where she held several positions including Medical Receptionist, Lab Technician and Personal Assistant.

In her free time, Crystal enjoys spending time with her family.

 

verla oaks, office assistant

Verla Oaks brings her 25 years experience in healthcare to Bluegrass Extended Care services as an Office Assistant, where she helps with office duties and patient care.  Prior to joining the BECS staff she worked as an Office & Surgery Coordinator with Lexington Clinic’s ENT dept for 10 years.  She has several years experience in other healthcare facilities including U.K. Healthcare and Lexington/Fayette Health Dept.

In her free time, Verla enjoys cooking and traveling.
